CAMP POLICIES & PARENT/GUARDIAN CONSENT
Please review the following information carefully before submitting your application.
1. Participation & Safety
KOA Camp includes outdoor activities such as hiking, waterfall trekking, kayaking, water activities, and other activities in natural environments.
I understand that outdoor activities involve inherent risks, including but not limited to slips, falls, changing weather conditions, water-related risks, insect bites, and other unexpected situations.
I understand that all campers are expected to follow the instructions of KOA staff and activity instructors for their own safety and the safety of others.
2. Health & Medical Information
I confirm that I have provided accurate and complete information regarding my child's health, allergies, medications, medical conditions, and any other information that may affect their participation in the camp.
I agree to inform KOA of any changes to my child's health before the start of camp.
3. Emergency Medical Care
In the event of illness or injury, KOA will make reasonable efforts to contact the parent/guardian or emergency contact.
If immediate care is necessary and a parent/guardian cannot be reached, I authorize KOA staff to arrange appropriate first aid, medical evaluation, transportation, or emergency medical treatment for my child.
4. Transportation
I give permission for my child to travel in transportation arranged by KOA as part of the camp program, including transportation between the meeting point, accommodation, activity locations, and other program destinations.
5. Water & Outdoor Activities
I understand that the camp may include kayaking, waterfall activities, and other activities in or around water.
I give permission for my child to participate in these activities with appropriate supervision and safety equipment.
I understand that KOA may restrict or modify my child's participation when staff determine that doing so is necessary for safety.
6. Program Changes & Weather
I understand that activities, locations, transportation, and schedules may be changed, postponed, or canceled due to weather, environmental conditions, safety concerns, facility availability, or other circumstances beyond KOA's reasonable control.
KOA will make reasonable efforts to provide an appropriate alternative activity when possible.
7. Camper Behavior
Campers are expected to respect other participants, staff, facilities, equipment, and the natural environment.
If a camper repeatedly refuses to follow safety instructions or behaves in a way that significantly affects the safety or well-being of others, KOA may contact the parent/guardian and, when necessary, require the camper to leave the program early.
8. Cancellation Policy
I understand that cancellation fees may apply after registration is confirmed.
The applicable cancellation and refund policy will be provided as part of the registration process. I understand that refunds may not be available for activities changed or canceled due to weather or safety considerations after the camp has begun.
